The lingering psychological effect of the Civil War and the concept of "State's Rights" versus the Federal Government, in which states imagine themselves bravely independent against a tyrannical central government, is mercilessly exploited to benefit the pocketbooks of a few against the interests of the many. Religion plays a huge role in this. Tax dollars that would fund public hospitals are instead channeled into "Faith Based Initiatives," where the churches function as charities and hospitals. It keeps the churches very strong and keeps free or low cost "secular" public health care at a minimum.

It's basically a disaster. Americans and their supposed "religious freedoms" only have themselves to blame.
