Default From which century was the codex widely used?

Before the codex (book) was the roll.
Papyri rolls eventually gave way to books.
The Roman poet Martial is often cited
as a late first century codex producer.

However it appears that this early practice
was not sustained in the empire, and was
essentially abandoned until another later
century.

So the question is, from which century
was the codex widely used, and what
is the range of opinion on this issue?
